Your Rights as a Pet Owner:

  • You and your pet will be treated with the utmost respect and consideration at all times.
  • You have the right to discuss, accept, or decline any proposed diagnostic tests or treatments. Seeking a second opinion is also your right.
  • You will know who is providing care to your pet and assured that such care complies with professional and legal standards.
  • Your constructive feedback is welcomed and will be thoughtfully considered to improve our services.
  • Your pet’s and your personal information will remain confidential, and access to medical records will be granted upon request.

Your Responsibilities as a Client:

  • Treat our staff, other clients, and patients with respect and consideration.
  • Arrive on time for appointments and inform us in advance if you need to cancel or will be late.
  • Provide as much relevant and accurate information as possible about your pet's health and medical history.
  • Adhere to the agreed-upon treatment plans and communicate openly with us about any concerns or assistance needed.
  • Share any concerns you may have about your pet's care right away so we may take steps to improve the experience.
  • Agree to fulfill the financial obligations related to your pet’s care.
  • Understand that veterinarians are legally and ethically bound to provide veterinary services strictly under the terms of a current veterinarian-client-patient relationship.

Our Hospital’s Commitments to You & Your Pets:

  • Provide high-quality medical care for your pet based on current veterinary practices.
  • Keep you informed about your pet’s health status and any procedures or treatments recommended.
  • Aim to minimize wait times and promptly inform you of any delays.
  • Present an upfront summary of costs for services and available payment options.

In these instances, we encourage open communication and understanding as we navigate through them together, always aiming to enhance our partnership for the benefit of your beloved pets. I have read and understand the terms of this Client Partnership Agreement with Chimacum Valley Veterinary Hospital. I agree to adhere to the responsibilities outlined in this agreement and understand my rights as a pet owner. I acknowledge that this agreement does not replace the legal and ethical obligations of the veterinary-client-patient relationship.
